  • Certificate of Employment with compensation

  • One month latest pay slip

  • Updated Employee Statement of Accumulated Value (ESAV) at least 24 months (HDMF)

  • Proof of billing (atleast two)

  • Bank statement (if any) - Checking Account

  • Income Tax Return

  • 4 pieces of  1x1 ID pictures

  • Marriage contract (if married)

  • Birth Certificate

  • Post dated Checks

SELF- EMPLOYED

  • 2 years of Audited Financial Statement with ITR'S w/ tax receipt business

  • DTI/SEC Registration

  • Mayor's permit (Current and last year)

  • Company ID and valid ID (Government) photo copy

  • One month latest pay slip/ Remittance

  • Updated Employee Statement of Accumulated Value (ESAV) at least 24 months (HDMF)

  • Proof of billing (at least two)

  • Bank statement (if any) - Checking Account

  • 4 pieces of 1x1 ID pictures

  • Marriage contract (If married)

  • Birth Certificate 

  • Post Dated Checks 

OVERSEAS WORKERS

  • Passport with signature (include pages of arrival and departure)

  • Job contract in the current employer

  • Certificate of Employment and Compensation

  • One month latest Pay slip/ Remittance

  • Updated ESAV at least 24 months (HDMF)

  • SPA Special Power ofAttorney (notarized or consularized)

  • Valid ID of atty-in-fact with Picture and signature

  • Proof of billing (atleast two)

  • Bank statement if any - Checking account / Remittance

  • 4 pieces of 1x1 ID pictures

  • Marriage contract (If married)

  • Birth Certificate

  • Post Dated Checks
